epidural /pca policy


I am currently reviewing/revising our pca and epidural pain management policies for post op pain management. I am asking what you do, don't do,ect,....
I work in a 200 bed facility, heavy on ortho, gyn,gu post op's that get pca's/epidurals. predominitly morphine pca's and fentanyl/ ropivacaine for the epidurals. one of our anesthesiologists like to hang a concurrent nalbuphine infusion with the fentanyl epidural,..........
what do you do? how do you monitor?, do you routinely use foley cath's with epidurals?
do you have any standards for breakthru pain, or low bp's?
