Shift report forms/organizing notes help needed

Hi! I am an RN with one and a half years of Med/Surg experience who just started on a hematology oncology floor with hight acuity patients. I am really enjoying the challenge, but I need help organizing my notes during the day to give an efficient and complete shift report. Most of the nurses on my unit use binders and tabs for each patient, which I am trying. Any suggestions or forms to download? I really appreciate it! Any other general advice for new hematology oncology would be appreciated as well

I also liked to use a flowsheet type paper when working in the hospital. I created an excel spreadsheet that contained spaces for pt name & room, dx, MD, diet, VS, lab, activity, misc. just whatever you find most appropriate for you. You can personalize it anyway you want. I also created a flowsheet for my medications.
