Need OHN depression screening forms

I am in my last semester of nursing... hooray, and will be going to a blue collar work site to assist with health screenings. My part is depression screenings. I would like to know what are some common depression screenings OHN utilize in todays work place?

Are any of the forms you suggest on the internet? Any advice provided for depression screenings would be welcomed.
