Staffing ratio for this patient

30 weeker, twin gestation, contracting q 2-3, dil. 1+,75,-2, BP's 145/85, +3 DTR's, On Mag.... 6gm load, 3gm/hr. When she was still contracting q 2-3... additional 2gm load and ^ to 3.5 gm per hour. Q 15min vitals. Q 2 hr mag levels. PIH labs. Q hour I/O and DTR's. Level II hospital w/ approx 100 del/month. Our closest level III is 100 miles away.

Just throwing this out there. I don 't want to tell the whole story until I get some feedback...

I am not a Mag level kind myself. Patient response is key, so diligent assessment is called for. I would like to know what this patients PIH profile showed.

NO WAY this patient would be 1:3 where I work. As SBE said, MAYBE once stablized 1:2. When I worked critical care we only had a 1:2 ratio, I can't imagine a 1:3 ration given the circumstances and the monioring of so many babies. I wouldn't want that kind of responsibility, in any level hospital.

Wow, I'm really sorry that happened to you, it sounds like one of those days/nights from the edge. I don't even know what to say except, "That really sucks", which I'm sure is not at all therapeutic! LOL!!

Certainly safety is an issue here. I can't make any judgment since I was not there. I hope it gets better for you, and the patients very quickly.

Well... this is just another event in the long line of short staffing issues at my hospital.
