newborn assesment

hi everyone! I need your opion. I admited a healthy male newborn to our MB unit, and everything was wnl...one thing I noticed was that the head of the penis was slightly curve. the color, skin and everything was normal so I proceeded to document it as normal genitalia. The primary nurse assesed baby again later on the shift, and said the penis looked curved and that it wasn't documented. Well I left work, forgot to chart the curved penis...should I just go back to work and chart it, I believe she might of charted it.

I don't think all penises are going to be the exact same shape...so I think you're good.

you should've asked a senior nurse or asked a doctor to take a look if you're not sure about something so that it could be properly assessed.

you can't really go back and "insert" your assessment at this time. when you get back, just refer this to your MD and do necessary interventions.

next time, be more careful. remember, not documented means not done. little mistakes like this could be potentially harmful.

thanks for your replies! I actually called the unit and spoke to the nurse taking of the baby. She assesed baby said it was normal, told peds and said it was normal..baby then got a circ.
