What is the standard for charting access? Do you chart the how high the accel goes by using a number or just a + signifying it is present? Thanks

- Jul 29, '12 by babyktchrThe paper charting SHOULD have a legend that dictates what to use. If Hollister is being used, they use + for a 10x10 and ++ for 15x15. Make sure you are using the denoted legend as they have written in policy or on the paper for itself. They also should be using NICHD nomenclature.
