    I'm a traveling nurse but work mostly in small hospitals that still have paper charting.
    What is the standard for charting access? Do you chart the how high the accel goes by using a number or just a + signifying it is present? Thanks

  4. The paper charting SHOULD have a legend that dictates what to use. If Hollister is being used, they use + for a 10x10 and ++ for 15x15. Make sure you are using the denoted legend as they have written in policy or on the paper for itself. They also should be using NICHD nomenclature.
