C section recovery

    I work in a small overseas military hospital. For those of you back in the states, do you utilize a separate wound care assessment note for your post op c/s or is it just documented in the shift assessment/clinical note?

    I just document under the general nurse assessment, and add details under nurses notes. If the incision is well approximated, CDI, except for maybe some small drainage... that's all WNL for a C/S.

    I document my c/s wound under the 'integumentary' section of my routine shift assessment. Our computer program has options for type of wound/dressing/drainage/measurements.
