Any suggestions? Thanks in advance!!
I use a report sheet provided by my unit, and staple it to a blank daily planner sheet from Outlook of the hours of my shift (e.g. 1900 to 0700). That way I have the info that I need on my couplets on one page, and I can fill in what I need to do at what hour on the other sheet like a dayplanner. The report sheet is divided down the middle with mom in one column and baby in the other and holds 3 couplets per side of paper. The mom info includes G/P, RH, GBS, HepB, Rubella, HIV, allergies, diet, date/time of delivery, type of delivery, complications, IV info, void/Foley, Fundus/Lochia, Meds Given on prev shift, meds due, and a small box for notes.
On the baby side includes apgars, weight, feed time, breast or bottle, void/stool, VS due, 18-hr BP due, PKU, TCB, SB, Hearing screen, Hep B, circ, cord clamp and a place for notes.
I am doing this from memory, so I may have forgotten something. Anyway, after taking report, I check the MARS and fill in my dayplanner sheet with meds/labs/procedures due at which time, then I have a to-do list according to time. Seems to help pretty good so far. I work tonight, so I'll bring home a blank report sheet and try to scan it tomorrow.

DTK babymom,
Can you send me a copy of your report sheet? I start on GYN in a few weeks and could use a heads-up.
Thanks.

Is there any way you can attach a copy of your report sheet for OB/GYN patients?

Please send me a copy of your mother/baby nursing report sheet. I am an NICU nurse that sometimes floats to FCC and REALLY need an organized sheet.

Here is what I use to get and give report.... during the day, I use a timeline, making sure to write down EVERYTHING i need to do, including things like recheck pain after pain meds are given, turn pt at every 2 hr timeslot, zero IV pump, etc.

Good luck!
