by incurable_optimism Jun 19, '12I usually don't have much trouble with care plans but this quarter all of our patients are on ventilators and for the most part are A&O x0. So my question is, what kind of subjective data can I have if my patient is unable to tell me anything? Because wouldn't anything else be an observation and be considered objective?

your vented patient if arousable can indicate plenty by wirting, squeezing your hand.....mouthing words. - Jun 19, '12 by GrnTeai have found that asking them to "write" in all capital letters on the palm of my hand, as i verify each letter, is very useful. most of these folks can't use a pen and paper, but most retain their knowledge of abcs.
