Risk for falls Care Plan

    Hey yall, I need some help!

    I'm working on a care plan for my patient. I'm having a hard time coming up with a nursing diagnosis (for some reason I just don't get them!) I chose Risk for falls.

    Patient info: 95 y/o with dementia and delirium, very combative, and sometimes the nurses have to give Ativan and Geodon. Is also hypertensive and on many different hypertensives..He sleeps all day and is very hard to rouse.

    Any ideas for a nursing dx??

    maybe disturbed sleep pattern r/t excessive daytime sleeping? maybe confusion either acute or chronic? these may work depends on how you interpret Carpenito-Moyet.

    or you could probably do risk for skin breakdown because the pt sleeps all day and is hard to arouse, and that sounds like the patient is bedridden to me.

    With the Ativan, it sounds like he's snowed to me.
