I need help in making a Postpartum Care Plan for a 24 year old patient who is drug dependent and have given birth to a baby with defects who is confined to a N ICU in the same hospital. I would appreciate a sample Postpartum Care Plan. Thanks.

Have you searched "postpartum care plan?" There may be some templates out there for you to use.

You want your care plan to be written using proper spelling and grammar. So it should reference a 24 year old patient who *has* given birth.

She was a 42 year old woman with a history of anemia and diabetic. She has Lochia Serosa and had episioto y to help in the delivery. Can somebody help in the diagnosis and intervention?

It seems like way too many people post on here like...here's the pt info...now what? Take some time to at least try and give what YOUR OWN thoughts are. Not just repeating the pt info and asking again for help. The people on All Nurses are so helpful, willing to put in their own time and effort into helping students and fellow nurses, but those asking for help need to do their part as well!!

Do you have a care plan book?? Also, is the care plan for the mother or the baby?? If not, here's an article that could help you create one.

Since the OP never came back to show what he has already come up with, I'm guessing he was never interested in learning and simply wanted the nice folks at AN to do his work for him.
