MY identified problem is :Risk for Constipation

and my Nursing diagnosis is:Risk for Constipation related to Insufficient physical activity and inadequate fluid intake

Pls tell me if whether my problem or diagnosis is properly stated.

yes, this is stated properly stated.

this patient has chronic renal failure, an infected fistula and has probably been hospitalized because they need to be on iv antibiotics, and has hypertension. i would have used
impaired tissue integrity related to inflammatory process as evidenced by nonfunctional dialysis fistula
[this addresses the reason they were hospitalized--the infection in the fistula, stated in nursing language]

hyperthermia related to inflammatory process
[this addresses one of the chief complaints--you don't know if the fever hasn't been eradicated] or
ineffective protection related to antibiotic drug therapy
[this addresses a complication of treatment because i guarantee this patient isn't just taking up bed space to watch soap operas--they are either going to treat the infected fistula or the chest pain]

a "risk for" diagnosis is not an actual nursing problem that exists. this patient is sick enough to have some actual nursing problems. they are found by looking at the symptoms of what is going on and considering the treatments that are being ordered for the patient. that is why part of one's assessment activities should include:

    • a physical assessment of the patient

    • assessment of the patient's ability and any assistance they need to accomplish their adls (activities of daily living) with the disease

    • data collected from the medical record (information in the doctor's history and physical, information in the doctor's progress notes, test result information, notes by ancillary healthcare providers such as physical therapists and dietitians

    • knowing the pathophysiology, signs/symptoms, usual tests ordered, and
      medical treatment for the medical disease or condition that the patient has
      . this includes knowing about any medical procedures that have been performed on the patient, their expected consequences during the healing phase, and potential complications. if this information is not known, then you need to research and find it.

all that information helps in deciding what the patient's nursing problems and nursing needs are going to be.
