Does anyone have a WORD doc. version of a peds H & P that they could post. I'm supposed to do one on a child (my nephew) for my nursing school, but we don't have a form to use. It has to be a full H & P (except no genital exam) and I haven't been able to figure out the best way to do this. I'd appreciate your help.

- Mar 7 by SUNFL0WERHere is a rough draft. You need to use descriptive terms (i.e. smooth, bulging) and if something that isn't there that shouldn't be anyways, I always note that it is not there. Also, remember that there should never be the word "normal" in an assessment because normal is different for everyone.
