A new order for Vancomycin 1250mg IV daily is received. A 1gm vial of Vanco is reconstituted with 20 ml of sterile water. What is the total volume of Vanco that you would add to a 250ml bag of NSS?

Dose Desired: 1250 mg

Dose on Hand: 1 gram/20 mL

Conversion Factor: 1 gram = 1000 mg

20 mL/1 gram (dose on hand) x 1250 mg/1 (dose desired) x 1 gram/1000mg (conversion factor) = 25 mL

Analysis
: you will need to reconsitute 2 vials of the Vancomycin in order to add the required amount to the 250mL bag of normal saline.
