How to use the nursing process

Ok, so here is the nursing process. Assess, diagnosis, plan, implement and evaluate. How do I know if a question is a nursing process question? How do I use it? Can anyone provide an example of a question that requires this nursing process? I'd really appreciate if someone can take the time to do this for me, it would make a big difference to this nursing student who will start in the fall.

What is tachypnea? High RR. Oxygen is transported via red BLOOD cells. If a person is breathing faster than normal, it could be because they're not receiving enough oxygen to the cell so they compensate by breathing more frequently in an attempt to get more oxygen. But if their oxygen saturation is 100 what is the problem? Could be a problem with perfusion, in which the BLOOD is not adequately delivering the oxygen, or the person is bleeding or has anemia. There could be other reasons.

Hypotension means low blood pressure. What does that tell you? Could be because of decreased vascular resistance, bleeding, shock, and many other things. But you should understand that it has a relationship to BLOOD.

All three are involved with things that are part of the cardiovascular system so it should set off an alarm that something might be off. If a person is having abnormal heart rhythm, which tachycardia is, you want to see what's going on.

Op I'm going to be frank with you, reading into your post history it brought some concerns into my mind. I am glad that you are continuing your dream of being a nurse but as this is your second chance I sincerely hope that you realize that you have to take responsibility for yourself. I am saying this because judging from your post in which you believed females to be more motivated or you failed because you weren't used to nclex style or even the fact that you didn't know nursing school was going to be so hard, you seem to always place blame on something else. I commend you for your perseverance and determination but remember that you need to identify your own faults in order to work on them. It seems like you have the tendency to use rationalization(psych defense mechanism) to justify why you didn't do well. If you think females are more motivated, maybe it's you that is not putting enough work in, then you need to put in more effort. If you weren't used to nclex questions then it behooves you to get used to them and practice till you do. You don't wait till after you fail the class, you do it right away. You didn't know nursing school was hard, well after you found out which should be before you failed, you should've went to your professor outside of class to identify what you can do. Put in extra work, don't wait till you fail to say it's hard and I wasn't used to it. To tie it back to nursing, after you assess a problem and come up with the diagnosis, what is your plan? What is your intervention?

Please understand I am not trying to attack you but rather I hope to enlighten you so that you can succeed in something you clearly have passion for. I apologize in advance as my assessment on you could totally be wrong as I'm basing it on a few posts I've seen from you, but try to stop and think about it in case it is right. You have a second chance and that is remarkable. Good luck, I hope that you work hard and do well in your goal.

Hey, OP, really. Stop this right now. You are going to go to nursing school (a second time) to learn all this. You can learn it there. You'll learn how to know that BP, blood loss, and heart rate are connected, and why, because you'll take anatomy and physiology. Kylam has given you some terrific advice. You are not going to waltz in there, having boned up on "What's a nursing process question? What do I do with one if I identify it????" and cruise.

Nursing school is work. Lots of adult things are work. If it's not nursing school, it's gonna be something else. So get some help getting your head together on learning how to look at the world through self-respecting, self-confident, and self-motivated eyes, or nothing is gonna work for you. Least of all, nursing. Not just school, NURSING.
