how can i write a focus note on a newborn?

some conditions do not declare themselves immediately. you can look in your text to discover a few. therefore, your charting notes all your normal findings, which demonstrate that no bad things have developed. you focus on what has happened that is supposed to be happening; you say how you know this is a perfectly normal
newborn, if that makes any sense.

Every nurse has her own style on what they write for an admission note on a newborn. Some include a lot of details while others write just the basics. Im assuming the newborn is in well baby nursery and not NICU. I would look at other nurses notes on admission to get an idea of what they include in their note. This is an example: Newborn FT baby boy/girl, weight 3600 gm. admitted to WBN. Can include that you identified the baby with a particular RN. Active, alert, pink color. No respiratory distress noted. HR..., RR..., Temp. (Rectal/axillary)..., BP..... , Dxt..... No void/meconium passed. No visitors at ths time. Will continue to monitor.
