Help With Nursing Care Plans

Thank you for you'r reply. I did my care plan on Impaired dentition r/t ineffective oral hygiene AMB Halitosis, excessive plaque, enamel discoloration, tooth fractures, premature tooth loss of primary teeth, cliet stated he had not brushed teeth in 2 days.

That works, but it sounds like it is for one individual patient. You gave the impression that this was a care plan for a community which to me means a group of clients and not just one. I also thought you were going more for a teaching strategy to work into the plan.
