General question about how to write a case study

I'm in my 2nd semester of nursing school and I'm having a little difficulty with writing my case study. The part that I am having difficulty is intertwining my patient with the patho. What I noticed that I'm doing is first writing about the disease and then writing about my patient after wards. How can I intertwine it so that my patient is not an afterthought?

Here are some general info. I'm writing about DM type 2. She was admitted for uncontrolled hyperglycemia (BG 460), nausea, vomiting, loss of appetite. She has other comorbidities relating to DM.

I'm not sure if my question is too general, but I thought I would ask to see if I can get some good tips.
