effective nursing intervention

im doing a care plan.so far so good-i think. my problem is my Evaluation section. whats the difference between "Effectiveness of Nursing Intervention" and "Attainment of Expected Outcome."?? my brain won't separate the two. to me, its the same thing, but obviously its not.any help would be so appreciated

The evaluation of goals section is where you decide if your patient met the goals you laid out for him previously in the care plan i.e., if one of your goals is "Patient will consume 250mL of fluids by 2pm." then your evaluation of that goal is to look at the patient's I&O up until 2pm and decide if they met the goal or not. Some goals can be partially met depending on what the goal is. Does that make sense?

It's a pretty good idea to keep a dictionary around.
  • effectiveness - producing a desired result
  • attainment - anything accomplished, or achieved
