Hi, I just need a little help with some wording on my nursing diagnosis (for now that is).

I have a patient who had a stroke and has recovered well... I want to use Knowledge, Deficient but am unsure of how to word my r/t statement. The patient pretty much just needs to be educated on the symptoms of a stroke and the importance of getting help quickly if those symptoms should occur again.... any idea of how I can make that my r/t statement??? Thanks a bunch!

The taxonomy gives you advice on the related factors to use for this diagnosis. The related factors are:

  • cognitive limitation (in other words, not the brightest light bulb)
  • information misinterpretation
  • lack of exposure (to information sources)
  • lack of interest in learning (apathy)
  • lack of recall (bad memory)
  • unfamiliarity with information resources (doesn't know where to look)
  • lack of information

But where do you get your taxonomy information from??? I am currently looking in my Taber's 20th Edition and it has a WONDERFUL nursing dx section in the back... it is generally very helpful and makes it very easy to

1) pick out the nursing dx

2) come up with the correct r/t statement

3) find the best as evidenced by or as manifested by statement

4) Ensure that I am picking the right dx

BUT... it did not have "Lack of information" as one of the Related Factors. The wording sounds simple enough... but for a new NS, working on my first care plan, I had no idea.

I have files of certain diagnoses of which Deficient Knowledge is one. I have the related factors I commonly use for it listed in that file. "Lack of information" is something that I picked up somewhere and added. Don't remember where, but it fit, so I added it. Half the battle with some of these diagnoses is the wording we have to come up with, isn't it? The other file I often have to open up is Ineffective Health Maintenance.
