Mrs. Smother has recently had an ostomy colon due to exacerbation of unlcerative colitis. She is relieved that portion of her bowel will be rested, but is quite uncertain how to care for her new ostomy. As the primary nurse, you notice that Mrs. Smither's colostomy has leaked and needs to be changed.

what assessment would you perform?

what assessment would you perform?

Patient doesn't know how to care for it and the ostomy bag needs to removed and changed. What do you need to physically assess now that the bag is off? What non-physical assessment do you need to do?

Okay, what system do you need to assess if stool gets on it? What may happen to it? Why is this situation potentially dangerous for that particular system? That should get the brain juices flowing a bit.
