    looking for any helpful ideas on how to create a system to document the effectiveness or adverse effects of all new medications started in the hospital. at our hospital we have to address any new medication started within 12 hours of the first dose given, however this is very frequently missed. on our medication record we stamp "New Medication" right on the MAR and still this is not being addressed. any ideas for improvement whould be very much appreciated!
