I read a nursing note that said "strong urine smell of vegatable soup.." I know this must be an indicator of somethin.. and answers??

I've never heard that one either. I was taught to only use definite descriptive terms...0.5 cm laceration, fruity odor, etc..."vegetable soup" can mean a lot of different things to a lot of different people...
