It does relate because these things do happen at hospitals whether they be medical or psychiatric. None of the articles were journalistic sensationalism rather a report of the breakdown in patient care.
When I was a student, working on female acute care in Psychiatry, having 25 new patients would have been mayhem!

And are you really saying that someone in the waiting room of an ED has not been triaged? Seems like the family attempted to get a nurse to look at her.


Patient triaged, designated level 2, one level up from immediate/urgent action. I would consider that serious.
Nurse did not follow reassessment protocol.

All I'm saying is it does happen.
yes, things like this happen, but #1 was waiting for a bed in psych, already seen...#2 was a known drug user, had already been seen more than once, and i repeat on #3 what else was going on in that ED, at that point in time.....are you going to try and tell me that you have NEVER made a med error, always been on time (on the dot) for all meds and treatments, and assessments? really? you want to go there?

In my posts regarding this incident I have never remotely suggested that I have never made an error. I have made med errors, been late on assessments etc. Knowing that, I know that these things happen and it is unfortunate. I am not blaming anyone just simply responding to the statement "that this would never happen in an ED" when in fact it does happen. There are many reasons why these things happen; human error, inadequate staffing, poorly explained protocols to handle different/unusual situations etc. Whatever the reason these things DO happen.

BTW you are changing your argument regarding #1, first you said not an ED now say she had been seen/triaged and was waiting for a bed. Whatever, what followed was egregious. Patient was ignored and died. Staff falsified records to cover their a*$ess. From what posts of yours I have read I feel it safe to say that even you know and can admit that what happened there was waaay wrong.

That's f'd up. I have been an ER nurse for years and in those situations, the security guard will defer to the nurse. I mean seriously ...I have often had pts who are melodramatic or crazy and lay on the floor ..but I still fully assess them...just to make sure. It DOES happen, that upon occasion that people stumble in to the ED who are ACTUALLY sick. Crazy, but true.

I laid on the floor of an ER here in Arizona, for 6hrs while coughing up blood. I was misdiagnosed with a PE and put on Coumadin. I actually had a ruptured artery in the lower part of my Rt. Upper lobe. The next time it ruptured I was still on the coumadin and could have died. The ironic part was that after waiting 6 hrs , walking around at times to the bathroom etc. I was put on bed rest in the ER. ???DUH???? I sat and watched as every child with a runny nose was seen before me. HOW DO YOU SPELL LIABILITY? I actually had people move away from me because they couldn't stand the sight of the blood I was coughing up. Don't tell me about how great the ERs are. I know you are faced with every kind of drug seeker, homeless, and psychos, but UNFORTUNATLY that comes with the territory. Truly sick people should be seen first! The staff at the window could see how much blood I was losing.

I must say, I find it EXTREMELY difficult to believe that ANY ER nurse would see a person laying on a floor coughing up blood and ignore them. If indeed that is the case, well then I am sure there is a very special spot for that nurse...in hell.

I would agree this guy may have been able to walk but was so sick he lay on the floor. However, whatever the cause HE SHOULD HAVE BEEN ASSESSED before being kicked out or whatever, period. Also, when did it become practice to allow security to decide who is seen and who isnt? Obviously, this man was very ill AEB he died!It is not like this situation has not occurred before. Remember about 2 yrs ago the poor lady laying in the ER floor and was kicked/hit by security b/c she wouldn't comply? and there she died right there. Emergency rooms have got to change their triage practices b/c of the mass amount of inflow they receive now days if they wish to avoid liability. There needs to be many changes made and wait times decreased.

I hope they get to the bottom of the situation and if errors were committed in proper procedure, they are found and corrected immediately so no one else suffers from the same. My heart goes out to the family of the man who was lost. This is truly a sad loss for them all.

He says he and other family members are still mourning Irving's death and did not want to talk on camera, but he did tell us about the run-in Irving had with the security guard at the hospital and says his brother got so weary after waiting more than four hours for the result of an X-Ray and blood work that was taken in the ER that Irving just decided to leave the hospital"


OK, i'm confused. This sounds to me like he was seen and an evaluation was at least started if he had had an x-ray and blood drawn. Perhaps he was lying in the floor and the security guard asked him to get up? The security guard certainly should have called for a nurse to check him but if he was blocking an area people are waiting in and trying to walk in I can see why he was asked to move. From this it sounds like he got tired of waiting and left-he was obviously already pretty sick and then continued to deteriorate after leaving. Hard to tell from the info given.
