Pharmacy is also responsible for double checking the order...pharmacy is looking for different things...they only look to see if the medication is safe...they have no idea if it's for the right patient unless you work in a facility that requires the physicians med order to be faxed or scanned down to the pharmacy on the original order sheet.

Pharmacy prints the label.

Pharmacy mixes the solution.

If the solution comes up with the label consistent with the original order...there is NO WAY that a nurse can know what is inside of it.

The nurse is NOT responsible in those instances.
And I personally know several pharmacists who will not work in a hospital because they can't eyeball an IV bag and know that the contents match the label. One of them was a friend of mine who simply couldn't check anything she hadn't prepared herself, and has mostly worked in mail order and now retail.

What happens when people are jailed for someone else's mistake?
people who are smart will quit practicing

UHHHH- that just leaves the un-smart to take care of us

You said it. How can a pharmacist know the acutal contents of an iv bag he did not mix himself? Does a pharmacy degree endow you with magic x-ray vision, or do you have X-men type smell like woverine so that you can smell the concentration of the iv. Yeah maybe thats it!
