Drive-Thru ER Concept Studied at Stanford

So, how does it work? Here’s a description of the concept in action:

The volunteer patients made their way through the drive-thru triage as though they were being seen at the emergency room. As cars entered the parking garage, patients registered and were given paperwork. They then drove through one of two lines and stopped at the first station, triage, where nurses and emergency department technicians checked for vital signs — temperature, heart rate, blood pressure and respiration — and gathered the patients’ medical backgrounds. Doctors, nurses and other medical staff wore gowns and gloves throughout the exercise.

From there, patients drove another 10 to 15 feet for a medical screening exam, where doctors reviewed the symptoms and made a diagnosis. Finally, they were discharged or admitted to the hospital.

I think it's a good idea, if we're going to have nonemergent patients anyway. I'll bet money that diagnoses will be missed because the patient was not fully undressed or it was hard to do a thorough exam. Abdominal issues, for example. I would also predict that there will be a line of cars around the block, creating a parking lot issue. Unfortunately we'll also have dedicated staff to those nonemergent drivethroughs. If we made them go through the regular process we could triage sicker people ahead of them. With a drive through the sick people are left waiting.

I would think the sicker people would somehow be directed to go inside.

Do the people get out of the cars?

The concept though is kind of...strange. I'm sure people thought drive-through food joints were too. And drive through weddings. And funerals. Well I still think the funeral thing is a little weird.

If they do something like this along our highways and interstates I'm all for it. I think there would be a market for something like that. In cities no way.

We've discussed, in humor, the idea of a "drug vending machine". Just put in your money and select what you think you need. PCN, Toradol (would always be out of date), Lortab (would have to have att least half the slots in the machine), Bactrim, whatever. We've said we'd just put it in the ER driveway and let people just pull up and get it themselves.
And we are the fattest country in the world?? Surely you jest!! We won't even have to get out of the car to see a doctor....

seriously though i see it being a disaster how can a Dr thoughrly assess an ill pt and even worse what if the pt codes:stone

Simple. Just pop the hood and grab a set of jumper cables.

This is really a strange concept. I can't imagine any kind of meaningful assessment being done this way. This is little better than telephone triage.

I was just thinking that if they make a little tunnel, just like the carwash tunnel, then it'll give the patient privacy and enough room for the MD and nurses to undress and assess the patient. What do you guys think?
