I don't know. Those are nice sized preemie twins.

Wait! Were these flushes??? Who is still using heparin flushes on their unit?? Wasn't there a similar mishape where babies died because of heparin flushes??

That's what I'm wondering, too. Maybe an art line flush? I still can't figure out the timeline, though, since it shows him carrying them around in carseats. Maybe they got jaundiced and had to go back? But then they shouldn't have needed an art line or anything other than bili lights. The article also says "babies usually receive 10 units of heparin..." like this was some standard practice, like VitK and erythro. Something weird is going on here. Edit Sorry, dawn, didn't see that you had already pointed this out.

And if 13 babies got it because a tech had put the wrong vials out, then either a lot of nurses weren't checking vials, or someone was using the same vial for multiple patients. Baaaad practice. I've also never personally drawn up heparin, except to anti-coagulate just prior to starting ECMO. All other times it's done in the pharmacy. But the article says the nurse did it, so... this is such a weird story.

Dennis Quaid's twins were given a accidental over dose of heparin.
Why cant the nurses check the label three times, like they are taught to in pharmocology class?
This same thing happened in Indiana last year.
I think nurses are awesome people...but this is EXTREME carelessness.
So why do they NOT check the label three times?

Some places do still use heparin flush; we are one.

We had a near miss w/this drug, the vials are unbelievably similar. One has a label in this color blue, the other has a label in this color blue. Honest, they are that close.

Despite the incident in the midwest and other near misses, the manufacturer continues to package them that way. One would suppose that continuing to report this issue to the FDA would have some effect, but apparently not.

A totally unnecessary tragedy for all concerned.

It's obvious that you have never been a nurse, so please refrain from the quick judgement.

It's easy to pull a med from the pyxis, where it's always been and administer it like you always do. Especially if the wrong med is in the wrong spot.

This is an error, yes. Horrible, yes. But I wouldn't call it EXTREME carelessness. I won't throw stones on an error that can easily be my own.

I believe it shows a picture of the vials in the Indianapolis thread. The are dreadfully similiar in color....
