I was referring to home births with lay unlicensed people. I admit I'm not up to snuff on all the differences between the different types of midwives...but I personally think it's a little irresponsible to deliver at home with someone who is under the radar as far as regulation and licensure go.

I should specify that I was not replying specifically to your post. I just thought it was so interesting that in this study the direct-entry midwives in North Carolina achieved excellent results at home with what was considered a higher-risk population, demographically. During the time of the JAMA study cited in my previous post, there was no national credentialing standard for direct-entry midwives in the United States.

Direct-entry midwifery in the U.S. has moved significantly toward greater standardization via the Certified Professional Midwife credential, which allows for more than one method of education but creates a single entry-level standard in terms of competencies, knowledge base, etc. The CPM credential has become the professional standard in states that have passed midwifery legislation specific to out-of-hospital birth. babyRN, the problem is that Certified Professional Midwives are not allowed to legally practice in all 50 states. They do not have to be nurses, but they are trained, certified, have to have continuing ed, and have their own licensing/certifying body (NARM).

Because of this, they are forced to go underground and fly 'under the radar.' Just because they are under the radar does not mean they are incompetent; it just means that in the event of a hospital transfer they have to choose between taking responsibility for the woman and risking their livelihood/facing jail time or just 'dumping' her at the hospital door, which goes against everything midwives stand for.

I personally think CPMs should be federally regulated and legal in all 50. In my state they practice illegally. A friend of mine had two of her babies at home with one and would do it again if she could.

You learn something new every day! And right under my nose as far as specialty nursing goes. I am rather curious and will do some researching on this on my own. But yeah, if they are regulated and do have appropriate education, etc, it doesn't seem right that they are considered illegal.
