How to write letter to BON explaining the charges?

I'm in the process of getting my documents ready to mail out to the Texas board of nursing. They require that I provided a signed and dated statement/letter describing the incident(s) that I am reporting to them.

How formal should the letter be? How detailed should be?

If possible, can somebody post up a sample letter?

Hi. Okay, I do not have a criminal hx, but I did have to write a letter to the Board describing what I put on my application (mine was mental illness/alcohol abuse). I simply dated it, addressed it to the executive director of the Board (the letter will be given to the board members working on the case as well, but many times there are staff members in the meetings), and explained who I was and why I was writing the letter (obtain approval to sit for NCLEX), then explained what happened from the beginning. In the next paragraph, I explained how I was doing better and pretty much why I deserved a chance. I also told them if they were hesitant about approval lay out stipulations that I would have to meet in order to get approval. I wanted my license more than anything, so I was willing to jump through the hoops. I am in a monitoring program now and hopefully, when approved to work, I can find a job. But, just explain the situation, how you have changed/where you are now, and ask them what you need to do to get approval.

I just completed the process and all I did was list the charge and the date and a few sentences as to why I was charged with this. I recieved my approval letter last week.
