Written up for false documentation...

  1. 2 i work in ltc and today i was written up for false documentation. i documented in the tar that a treatment was done when it wasn't. when questioned by the supervisor i admitted that i did not do the tx d/t patient being asleep. however, i forgot to chart it was not done on the day it happened therefore it is false documentation. i am petrified that this may be taken to the bon and my license be taken. this is a serious offense and i'm sick over it. i didn't get fired today but i feel that it's coming. this makes my 5th write up and i'm still in my probation period. i'm seriously considering putting in my two weeks before i get fired.
    i have seen several nurses and cnas get fired the pass two months i've been there. i've been warned by co-workers to leave this job asap and that nurses that work there often get fired and reported to the bon.
    ltcf expects so much to to be done in so little time with so little staff. every nurse should be guilty of falsifying records at this facility. i know for a fact that not every resident is toileted and turned q2h, i know for a fact that every cream is not applied every shift, i know for a fact that every tylenol is not given in the middle of the night when a resident is asleep. how do i know? nurses tell me all the time. however, i'm the one that got reprimanded today. i take full responsibility for my actions, however i know that i can't do it all. i'm one nurse to 30 residents, every day i feel like i'm putting my lic. on the line.
    i did write a late entry that the tx was not done on such and such day. i just hope and pray that this mistake did not cost me my career.

    My God, You have got to find a safer environment, not only for your licensure, but for your mental health.

    Know that when and if you stand before the BON, they don't care how many patients you had, or what the other nurses are doing. It is your responsibility to protect YOUR license. I AM NOT SAYING THIS TO BE MEAN!!! I WANT THIS TO WAKE YOU!!!!!!!!!!

    I WOULD RUN AWAY, I WOULD NOT EVEN COMPLETE THE TWO WEEKs THERE. WRITE ALL THAT UP ON PAPER ON YOUR LETTER OF RESIGNATION. WRITE EVERYTHING THAT IS UNSAFE, UNPRACTICAL, and UNETHICAL. WRITE WHAT OTHER NURSES HAVE TOLD YOU. WRITE ABOUT YOUR FEARS OF LOSING YOUR LICENSE THERE. AND HAUL BUTT. I WOULD TAKE THAT LEAP OF FAITH, THERE IS BETTER OUT THERE!!!!!!!!!!!!

    I agree one hundred percent. I know I'm far from being a perfect nurse, however I do my best. I make sure that for the most part my residents are well taken care of. In court they will never care that I had 30 residents, a fall, new admission, call lights ringing off the hook, family to deal with, stuff to f/u from day shift or what other nurses do. I'm going to take a leap of faith and put in my two week notice. I need the two weeks to carry me over financially.

    Oh thank you so much. I have faith that I'll find my place in nursing someday. I know I'm not the best nurse in the world but I'm certainly not the worse. I'm going back to school nursing for a while...

    I read my first post again, and think it's too harsh to convey what I wanted to say.

    I AM SORRY THIS HAPPENED TO YOU.

    I AM SORRY THERE ARE PLACES LIKE THIS, AND ANYONE CAN END UP IN A PLACE LIKE THIS, CULTURES CHANGE ALL THE TIME.

    I DO NOT THINK YOU ARE A BAD NURSE, LAZY NURSE, or UNSAFE NURSE.

    NO ONE COULD BE SAFE EVEN WITH 30 WALKIE TALKIE PATIENTs. I KNOW THATs NOT THE CASE

    MY WHOLE GOES OUT TO YOU AND OTHERS IN THE SAME POSITION. I AM SO GLAD YOU HAVE THE COURAGE TO BE THE MASTER OF YOUR OWN FATE, YOU HAVE THE RIGHT TO BE TREATED AS A PROFESSIONAL, and GIVEN A HUMANE ASSIGNMENT TO CARRY OUT EACH SHIFT.

    Whatever IS going on, I think your whole personal happiness is dependent on getting HELP!!!
    WE ALL HAVE SITUATIONs THAT WE NEED HELP WITH.

    I am Recovering Addicted NP. I wasn't shielded from addiction because of my sex, education, salary, or socioeconomic level. My Beautiful wife who I love so dearly, She couldn't shield me from my addiction.

    I MYSELF HAD TO COME TO TERMs WITH THE FACT THAT THIS ''THING" WAS GOING TO TAKE EVERYTHING I LOVED FROM ME.

    I was addicted to Opiates, and I started the addiction from my very on prescription, and it led to the HARDEST AND SCARIEST THING I EVER HAD TO GET THROUGH, BUT . . .I WAS NOT ALONE, YOU ARE NOT EITHER.

    I DON'T KNOW WHAT YOU ARE SUFFERING FROM, BUT I AM HERE TO TELL YOU THAT NO MATTER WHAT IT IS THEIR IS SOMEONE OUT THERE TO HELP YOU!

    YOU NEED TO SEEK THAT HELP.

    YOU DESERVE TO BE A HAPPY, RESPECTED, LOVED, and PRODUCTIVE PERSON. YOU NEED TO TAKE TIME TO FIX WHATEVER IS HAUNTING YOU, BECAUSE YOU DESERVE THAT CHANCE.

    I KNOW NONE OF THIS IS MY BUSINESS, AND I AM GOING WAY OUT OF MY COMFORT ZONE TO APPEAL TO YOUR DEEPEST SENSE OF SELF, . . .

    I'm not so sure writing that you are afraid of losing your license is a very good idea. Why let them know that? It just gives them power over you.

    I have never worked at a LTC facility as a nurse. I would never do that from the numerous stories I have heard. It just seems so unsafe. Good luck to you and hope you find a safer job. Don't question your nursing skills. Some days I have a hard time managing 4 patients depending on what's going on with them. We just do the best we can and learn from it. All patients are not going to get the best care 100% of the time. We're only human. Maybe if we only had 1 or 2 patients and didn't have an overabundance of charting. I do the best I can.

    I work LTC and I know how busy it can get. But don't ever document something that you did not do. It's a lesson learned. Had they wanted to, they could have terminated you for it. I work with 30 residents. If I didn't have time to do something, I just don't chart it.

    It sounds like you had a lot of problems surrounding your career and possibly personal life? It seems that you've had many issues in your previous jobs also. I think it would be in your best interest to find a position that is less stressful to you. It doesn't matter what the other nurses are not doing and documenting. What matters is your license and livelihood. LTC can be a very toxic atmosphere and it's easy to get drawn into the drama. I urge you to find a position and look within yourself to see what needs to change in your practice.
    Obviously I don't work with you or know all that's going on but maybe this is your wake up call and chance to change for the better. If you have personal issues that are carrying over into your work maybe it's time to be honest with yourself and get help with these problems.I hope it all works out for you and that you're able to use this as an opportunity to change what needs to be.

    It is unlikely that you will be reported to the BON, but not impossible. Most LTC facilities do not want the bother, it could be time-consuming on their end.

    That being said - you have learned a lesson the hard way.

    Do not be surprised if you are let go at the end of the shift you are working the day that you hand in your notice. Many facilities will do that as 'damage' control. They are concerned that an unhappy employee may do some actual damage to the facility, or otherwise might be a problem. They may even escort you off of the property.

    And keep your notice as short as possible - it is not appropriate to write scathing letter - that ensures you will be asked to leave immediately. Just "This is to inform you that I will no longer be available to work here at XYZ after --/--/----, two weeks from today. Thank you for the opportunity to work here."
