Written up for false documentation...

i work in ltc and today i was written up for false documentation. i documented in the tar that a treatment was done when it wasn't. when questioned by the supervisor i admitted that i did not do the tx d/t patient being asleep. however, i forgot to chart it was not done on the day it happened therefore it is false documentation. i am petrified that this may be taken to the bon and my license be taken. this is a serious offense and i'm sick over it. i didn't get fired today but i feel that it's coming. this makes my 5th write up and i'm still in my probation period. i'm seriously considering putting in my two weeks before i get fired.

i have seen several nurses and cnas get fired the pass two months i've been there. i've been warned by co-workers to leave this job asap and that nurses that work there often get fired and reported to the bon.

ltcf expects so much to to be done in so little time with so little staff. every nurse should be guilty of falsifying records at this facility. i know for a fact that not every resident is toileted and turned q2h, i know for a fact that every cream is not applied every shift, i know for a fact that every tylenol is not given in the middle of the night when a resident is asleep. how do i know? nurses tell me all the time. however, i'm the one that got reprimanded today. i take full responsibility for my actions, however i know that i can't do it all. i'm one nurse to 30 residents, every day i feel like i'm putting my lic. on the line.

i did write a late entry that the tx was not done on such and such day. i just hope and pray that this mistake did not cost me my career.

Don't apologize because there is nothing to be sorry for. If you feel nursing is for you, then there's your answer...and again, that was a question only YOU could have answered. Not me or anyone else. All I know about you is what I read here, so I don't know what you're thinking or feeling or doing unless you choose to share it with us.

But in all honesty, the pattern that you have been showing does beg for THAT question to be asked...and people may keep asking it if they keep seeing that pattern. Something to keep in mind while you're figuring out where your niche is, and IMO that question is something you shouldn't take personally.

If you were a troll it'd be pretty impressive...I mean, nearly 4 years and 4100+ posts before you were finally found out? ;) Usually trolls show their colors in 110 posts or less.

for the record, i would be willing to bet dollars to doughnuts that meriwhen ( ) doesn't mean "has a legitimate beef with a mean employer." i think she means "real."

False documentation is the rabbit management pulls out of the hat when they want to get rid of you.

I'm sorry it happened to you. I'll take you at your word that your error was innocent. A simple oversight on a busy day. When you're "caught" you feel slimy and sick. Like a liar and a cheat. For an honorable person that's a pretty crappy feeling.

Again, I'm sorry. I wouldn't wish false documentation on anybody.

The NH is aware it happens everyday. But they targeted YOU. I'd be looking for another job ASAP

Blessing to you, you are in my prayers! Never document something until atfter you have completed the procedure that way you can note the changes, etc on what you assessed.

If you need to resign, do resign. I don't think you should put the reasons in writing though. Anything that's in writing can come back to haunt you.

Yeah this is what happened to me. I didn't sign out a couple (of the 200) meds i had administered that shift - an 'error' most of the other nurses make as well. Although some nurses sign the meds out without giving them...who cares???? just as long as they got 'signed' for.

My advice to you is get out of there. Even if you don't get fired, you do not sound like you are happy where you are. You worked hard to get your license don't jeopardize it for some ******employer who thinks 1 nurse to 30 patients is acceptable.

Wow, every post I read from you makes me want to run to my manager and kiss her feet and thank her profusely for hiring me. I'm so sorry you work in such a heckhole of a job. It sounds like this place is a trainwreck.

That said, you see a lot of nurses cutting corners...that's their license, that's their problem. Don't look at what they're doing- protect your license!!! Their corner cutting will come back to haunt them eventually. Be it where they are now, or somewhere else...old habits die hard.
