Pressure Ulcers/Wound Care

Hi all- Am looking for any protocols or information your hopsital might use to prevent hospital acquirred pressure ulcers. Do you have a process or protocol? Please share

Everyone is assessed according to the Braden Scale and care planned accordingly.

In addition to the Braden Scale, if your pt is immobile we have a minimum q2h turn schedule.

I found the above online. It lists many of things which are part of our pressure ulcer prevention protocol.

Our Pressure Ulcer guidance and policies come from the National Institute for Clinical Excellence (UK)
