    If anyone hasn't read this, very insightful article. Goes to show that unfortunately mistakes happened to produce change. Although with the advent of evidence based practice and ongoing research I would hope that mistakes will become a rarity.

    The wrong site surgery thing on there: How does marking the left leg and operating on the left leg stop amputation of the left leg if on the x-ray the right leg (which is the bad one) was marked wrong as the left leg?
    I can see where it helps in other situations, but not that one...
