Hi....I was wondering if anyone had any ideas regarding a specific community problem in which nurses can implement change and action. Any ideas will be much appreciated....thanks so much

Are you asking as a student in community health? If so, perform your community assessment and your problem areas will surface immediately...no short cuts. Community assessment skills are just as vital as human assessment skills.

A huge community problem that has to do with older adults is Home Safety. We developed a Home Safety called "Rest Assured" We reviewed hospital data on falls and found that there was an increase in older adults falling at home. The program consisted of going into the home doing an assessment of their environment, providing immediate education, providing written recommendations and then doing a follow-up assessment 4 weeks later. The program was so well received in the community that the local law enforcement agencies worked with us and provided Beacon Lights that the older adult could insert in their outside lighting fixture. If there was an immergency they click the switch twice and the light would become a beacon. Neighbors and law enforcement knew that this was a signal was assistance.

Here in New Brunswick,Canada we are very fortunate to have Extramural hospital,or hospital without walls.This team is community based with a fleet of cars to go out and community nurse.These nurse do everything to shorten patient stays by doing dressings,bloodwork etc in homes to helping someone with terminal illness die with dignity at home.They assess every need for all scopes of nursing in the community!Maybe you could contact them?Good Luck!
