Lapse in new nursing career, family emergency, now what

Am looking for advice. As new grad (second career in mid-life) in 07, worked internship in Home Health Nursing six months when sudden family crisis/emergency. As result left my position and nursed mother until her death then daughter until present time. Have worked one day week in private physician's office as well since Sept. 08 through these times with family. Now advice anyone?
