consumer directed services

I have a question when I searching for home care job. I googled "home care in XX city". I went into one company's website, and there are two application forms: one is called "in-home services", the other is "consumer directed services". There are lots of information about them online, but I still can not recognize what is their difference? and I don't know which application form I should fill. really appreciate any help.
