Jessica, a nurse with a bachelor of science in nursing (BSN) degree, has been licensed for nine months. She resides in a large city in the Northeastern United States and has submitted nearly one-hundred job applications, but remains unemployed. She says, "I didn't think finding a job would be this hard, especially since all of the hospital websites have tons of job postings." The purpose of this article is to explore the reasons why some newer nurses cannot land that first nursing position.

My job was eliminated two days ago...as a direct result of the changes in reimbursement to hospitals due to health care reform that was signed into law during the Obama years. The reduction in federal spending is resting heavily on Medicare and Medicaid (CMS) cost cutting measures. When a hospital has approximately 40 - 50% of its patients’ care reimbursed by CMS, those cuts begin to hit close to home for nurses. With more than 35 years’ experience as a Registered Nurse, I was too expensive to keep, even when my job was to maximize reimbursement.

Late nights often get me wondering about life, death, faith and hope. This just happens to be my thoughts for the night. I hope that there may be one person it could touch. We just never know what is to come in our lives but I think that if we all have something for us to believe in, hope and faith will show you clarity of your beliefs.

But hearing someone pronounce the words that will change your life forever is only the beginning; now the real work begins as you come to terms with the loss of the person you thought you were, and face an uncertain and unknowable future.

Presently I work as a home health/private duty nurse with medically complex/fragile pediatric patients. Most of my patients would require medical day care or inpatient placement if not for skilled nursing services. Many of my patients have fair or grim prognosis for their recovery and/or development.

So do you treat the monitor or the patient first? I have to say this patient taught me a lesson by using humor. Do you run all night and fight the monitor or do you assess your patient first? In this one experience with my new admit I learned a valuable lesson with humor infused.

Make life worth living- and let it live on in others that stay here long after we're gone. We just never know where the story begins or where it ends- but all that matters is that it is carried on with a legacy of love and family.

If you want a room full of nurses to become very quiet, mention working on a psych unit. Some will express interest. Others will quietly edge away. But neither group is likely to be aware how "normal" psych has become and just how many people actually have a diagnosable disorder.
