The Hospital & Healthsystem Association of Pennsylvania is again participating in National Healthcare Decisions Day on Thursday, April 16, 2009. The National Healthcare Decisions Day Initiative, now in its second year, is a collaborative effort of national, state, and community organizations committed to ensuring that all adults with decision-making capacity in the United States have the information and opportunity to communicate and document their health care decisions.Information for Providers
Thank you for participating in Healthcare Decisions Day. Here you will find a collection of documents for your own use as well as handout for your patients/residents.
If you have not already done so, please complete the online commitment form to let us know of your participation. Surveys are due no later than Friday, February 27.
