If you were in a voluntary program which kept your license clean, there is no need to disclose this info when interviewing, and I strongly advice you not to disclose when interviewing.
On the other hand, if your license has public record of discipline, you need to be upfront and sell yourself. I would also advice that you reapply to all jobs, and hope for the best. I would not assume that all HR departments keep track of people that they did not hire. and doubt that they would all document why you were not hired.
