hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

Srg1168Sorry for the late reply.. i had emailed you. Did you already called your licensing analyst? They usually put their phone number on the letter that they sent you so you could contact them directly. You could explain to them that you had misunderstood the question and ask for a new copy.

Nurses, how did you filled-up the #5 date started and completed, and the number of hours worked if you only put only 2 hospitals?

Since we have 4 semesters of clinicals, did you put the date starting from ncm 101 to 102 with 816 hours (408+408 hours, since we have 408 hours per semester) in hospital A?

and on hospital B, from 104 to 105 with 816 hours?????

I have questions what is the different between the nursing declaration education/ licenses and nursing recod experience. I'm about to apply my LVN in cal but I'm confused about this pls. Anybody will help have no idea. Thank you

Declaration is a form that contains information about you nursing education or license.Record of nursing experience is about your paid bedside experience for at least 24-48 months.

Hi dragon_lady...the hours that you mentioned which is 2880 those hours are both theory and clinical, right?

My school didnt want to breakdown my pediatric and maternity. They also doesn't want to list the total hours to my theory and clinical. I dont know what to do anymore after explaining to them over and over. The bvnpt abalyst told me that they just cant accept it if the dean refused to breakdown the hours.. :((( im so frustrated with my school since its a simple request and they cant do it.

That's their job,they should do it.After 4 years of hardwork,sacrifices and paying their expensive tuition fees,that's the least they can do for you.So selfish!!!!:(

I know..its so sad. The lady who handles this requirements doesnt want to listen and didnt even care to the letter bvnpt sent me when it was showed to her. She keeps on telling that it was integrated and the total hours was already in the transcript. I tried contacting our dean but the same lady was so rude and always say that the dean cant take calls. As if she doesnt want me to speak to the dean. :(

You know what, you should talk to your dean personally. Your dean should know how to list the breakdown of your RLE and the total hours. Don't bother to talk to the lady who handle the requirements, your dean, he/she should be the one to talk to you since you graduated from their program. I mean, if you insist on calling your dean, you'll get what you need. My school's like that. They don't easily entertain such requests. Thank God I have good networking inside the nursing office of my school so I can easily talk to my dean and the secretary. You should call em. Even if it takes you forever, cause that's what you need. It's your future that lies in that breakdown of subjects / RLE :) Good luck!

Hi mate do I need to pass this 2 forms with me?or just nursing declaration don't have any experience yet except my RLE back in college.
