CVS in NYC

hi! everyone. I am planning to apply in NYC for the RN examination. but i earned my nsg education outside the US. Philippines to be exact.

I just got denied by the California BRN because they found that my nsg education is defecient with MS nursing and the clinical. so they need me to retake it again (MS nsg with the clinical duty) before i can be eligible and be granted with the ATT.

so now i am sort of re routing my plans to NYC.

and i just have a question to make. because i got confused with all of the infos that i need to put in the CVS (cgfns) form.

1. do i need to put all of my educational background from elem-college?

2. and if i got 1 school i went when i was in middle school and continued it in the Philippines (which was converted to highschool already since that time there was ni K-12 yet) do i need to put both of them? or only the highschool?

3. i attended two schools for my BSN. do i need to put both of them as well? or just the one where i have completed and earned my degree from?

4.will it be a big deal if the infos of the date on the form does not match the infos date which my school will be sending them?

please do enlighten me as i am very slow to this kind. thank you so much!
