    I think they should help you. Are there MD's on staff who could do it?

    Google it, then augment it for what you want, then they sign off on it. They may very well already have one. Good Luck.

    Look to your states APN organization for guidance specific to your state.

    Ouch. Idk. But I always thought that's kind of their responsibility unless you start your own business.
