    Just a question: how would you know for sure the overdose caused the seizure?

    I'm not sure about the legalities. I was told by an instructor to never report errors to the patient, as this immediately incriminates everyone involved.

    With that said: I think patient should have a right to know if an error was made and steps done to correct it. We need to be accountable.
