Anticoagulation(coumadin)management services in NC

Does anyone know where there is a list of certified anticoagulation providers for the state or USA as a whole?
If anyone has worked on floors/ departments where coumadin therapy is initiated, who managed it whilst in hospital and then post- discharge?
I'm talking about people starting on coumadin/LMWH for VTE and also Atrial Fibrillation ie stroke prevention.
