Feeding Protocols

    We are looking at putting a feeding protocol of sorts into place to guide advancement of our preemie feedings. This will be a combined effort of nursing staff, OT/PT, and lactation. Does anyone use something similar to a protocol at your hospital? What do you use to determine feeding readiness/time for advancement? Any examples of experiences or paperwork would be great! Thanks!

    Do you mean as far as PO feeding goes? We recently instituted a cue-based feeding protocol. We use a feeding readiness scale developed by Susan Ludwig, OTR/L and a feeding quality scale developed by Kara Ann Waitzman, OTR/L. Our lactation consultant, in conjunction with our speech therapist, developed an algorithm to use for cue-based feedings. If you want to e-mail me your contact information, I will be happy to send you a copy of that, along with the clinical pathway explanation sheet.

    Please send me an email with the information you spoke about. In my unit the doctors are in charge of feeding schedules and po trials.

    Dear NICURN and KATRNC,
    I was just reading your answer to the above question. You were saying that you implement a Feeding Readiness Scale, a Feeding Quality Scale, and an algorithm to use for cue-based feedings. We are currently in the process of implementing these types of things in our NICU as well. Do you mind emailing me a copy of the above information as well so that we can get something objective in place for our babies?

    One of our NNPs developed a great feeding protocol that the nurses "order" everyday after calculating volume based on mLs/kg/day. We have the same stops and starts with the micropreemie, but it has made it very easy because it is nurse-generated.
    Readiness to feed is sort of individualized and different from attending to attending, but basically we feed EARLY unless the baby is on pressors or Indocin, or is having a lot of As and Bs/decompensation. They are divided into 3 protocols based on gestational age at birth.
    I can get a copy, but I assume I must ask someone's permission to "share". Also, how would be the best way to post these?
