Report Form

Hi everyone! I was wondering if anyone had made up a report form to either give or take report from on either level 2 or level 3 babies. I start in the unit soon, and would love to see what other people are doing. Thanks so much!

The best report form is one that follows your train of thought. Write info down as you get report and figure out what works best. I have a very OCD co-worker/friend who made hers on an excell spreadsheet that is about 5x7 and keeps a stack in her back.

I just write down the urgent stuff during report because I can't listen and write.
Then go back and make my list of important things to do. I make boxes for each hour, like a calendar and write what I need to do for that hour in the box, then check it off when I am done. I post it at eye level so i look at it often.
I started this method about 6 months into my practice, and 6 years later it is just habit. I could not survive ECMO without it!
