Programs with less than a 70% pass rate are required to appear before the BoN with a remediation plan to raise their student's passing rate or risk losing board approval.
There are private for-profit and public schools. Tuition ranges from very affordable to more than some associate's degree programs (I know of at least one that starts
at $30,000 for a 11 month practical nursing diploma, and that doesn't include books, fees, uniforms, and clinical requirements). The schools should be forthcoming with admission requirements and tuition and fees. There are schools throughout the state and it's probably easiest to search for a program close to where you live rather than have to worry about room & board plus transportation costs if you choose to attend a school out of the area.
