Hypersonic saline

  1. I work in a mixed unit but we don't get much neuro. When administering hypersonic saline s/p TBI, where do you want your serum sodium to be? I've seen 1 where they wanted it above 145.this makes sense to me because it would create the gradient you want and decrease edema. But I have a pt now that has an order to hold for sodium greater than 145. Thoughts?

    I work in a Neuro ICU in New Orleans. Normally, we want our sodium levels to be 150-155 to decrease cerebral edema. Typically with TBIs a NCC provider may want to decrease the hypertonic saline to assess for cerebral salt wasting especially if you've been consistently titration the 3% Saline and haven't seen a significant increase in the serum sodium and have an increase UO.
