A nurse is attending to apt who had just returned from surgery. After 20mins, the nurse observe the pt trying to pull out his IV line, get out bed and removing his wound drainage. what will be the best action the nurse should take
1-apply restraints jacket on the pt and document reason on the pt chart
2- call the charge nurse to request a sitter to be assign to the pt quickly
3- reorient the pt and make sure call light is @ pt reach and move the pt to the nursing station
4- put the jacket restraint on the pt and place the pt on wheelchair

4- compare apical and radial pulse :The apical and radial pulse should be compare to determine pulse deficit and these procedure is done by two nurses.

2- MI
3- magnesium toxicity-if you know what the magnesium sulfate is used for, you will eliminate option 2 and 4 because they have similarity. while option 1 has nothing to do with the medication. option 3 is the only answer that correlate with magnesium sulfate. The stem of the question do not indicate the pt has any medical problem.
4- Angina

how often should the nurse check and remove restraint: check every 15 minutes and it should not be on for greater than 24 hours

how should the nurse care for a newborn whom mother is drug addicted
1- frequent feeding
2- tight blanket wrap
3- stimulation
4- loose blanket wrap

what treat is Terbutaline sulfate used for: Hypertension Terbutaline is a bronchidilator.
what precaution isolation is for pt with meningitis:strict isolationDroplet precaution

what is the MOST complication of liver biopsy: hemorrhage
