NCLEX retest...Need support

Hello, I am hoping to find much support while I prepare to retest for the RN NCLEX exam. I am quite nervous. I took the exam almost 2 years ago, have not studied since as I was traumatized from my experiences of nursing school! Who can relate? I really need to retest. I want to do things right and send everything in properly. Do we need to resend all of the application forms? Like the form from the school we graduated from? I am so confused! Your help is much appreciated! I am a resident of WI. Thank you!
